Birth and Death Records

The Vital Statistics Registrar is responsible for recording all births and deaths in Shelby County. Records are available dating back to December 1908. 

Sidney-Shelby County Health Department - Birth and Death RecordsTo make it easier for you to request birth and death certificates, a form has been provided on this page for you to print.  You may fill out the form online or mail it, along with the required fee in the form of a money order or check, made out to Sidney-Shelby County Health Department, and a self addressed stamped envelope to:
