Sidney-Shelby County Health Department CMH
(Children with Medical Handicaps)

Sidney-Shelby County Health Department BCMHChildren with Medical Handicaps (CMH) aids families of children with potentially handicapping conditions and with special health needs. CMH can also assist with treatment of these children by appropriate health care providers.  The general program is divided into the Diagnostic Program and the Treatment Program.

The Sidney-Shelby County Health Department provides the services of public health nurses who can assist Shelby County families with various aspects of the CMH program.  This may include activities such as:  help with application to the program, referral to appropriate physicians, referral to other agencies who may provide services or assistance, explanation of the CMH program and letters of approval, among other services.  For more information or assistance, call 937-498-7459 or 937-498-7339.
