Clerical and Vital Statistics Division

The Clerical Division of the Sidney-Shelby County Health Department make the appointments, answer phones and take care of the everyday paperwork and requests that come into the department. Other clerical duties of the staff are keeping daily work records, issuing licenses and permits and recording appointments for the Well Child Clinic and the nursing staff.

The Vital Statistics Registrar is responsible for recording all births and deaths in Shelby County. Records are available dating back to December 1908. To make it easier for you to request birth and death certificates, a form has been provided on this page for you to print. Fill out the form and mail it, along with the required fee and a self addressed stamped envelope to:
